When booking an Initial Consultation Appointment for Chiropractic, you will have the opportunity to either fill in your intake forms in person at our clinic when you visit, or alternatively, you can fill out your forms in advance, online. If you do fill out a health screen form after booking an appointment online, your information is sent securely through our booking system and will be reviewed upon receipt by your selected Practitioner. Information you provide in your Patient Intake forms allow us to keep accurate and up to date records and helps us to assess your musculoskeletal health.
