New Patients and Mandurah Health
Your First Appointment
If you have X rays, relevant medical information and any medical referrals, please bring them along to your first appointment.
Please arrive at least 15 minutes before your scheduled appointment so that you have adequate time to fill out your New Client Form.
We recommend that when making a follow up appointment, you book ahead to avoid disappointment. If you are unable to book your preferred time, then please ask our Chiropractic Assistant team at the front desk to place you on our waiting list. We can then call you if a space becomes available.
Running late or need to reschedule your appointment? We ask that you let us know by ringing our reception (08)9535 2303.
